Qualifications:

  • Education: Master's Degree in Social Work (MSW)
  • Licensure: Active NY State license
  • Experience: Minimum of 1 year of experience
  • Technical Skills: Proficiency with electronic health records (EHR).
  • Soft Skills: Empathy, cultural competence, and a patient-centered approach.


Key Responsibilities:

  • Conduct comprehensive psychosocial assessments and diagnoses for patients and their families.
  • Provide evidence-based individual, group, and family therapy.
  • Develop, implement, and monitor individualized treatment plans in collaboration with a multidisciplinary healthcare team.
  • Perform crisis intervention and risk assessments, implementing appropriate safety plans as needed.
  • Advocate for patients and families to ensure access to community resources, social services, and financial assistance programs.
  • Provide care coordination, including patient education, discharge planning, and referrals to appropriate follow-up care.
  • Maintain timely, accurate, and confidential patient records in compliance with all state, federal, and organizational policies.
  • Participate in case conferences, team meetings, and ongoing professional development opportunities.
